  2. MRSA or MRSE
    DOC:
    Vancomycin +- gentamicin or +- rifampin
  3. Streptococcus Pyogenes
    Group A C and G
    DOC:
    Penicillin or amoxicillin

    Alternative: Erythromycin (macrolide) or clindamycin or first gen ceph

    note:clarithromycin is not recommended in pregnant women
  4. Streptococcus agalactiae (group B)
    DOC:
    Penicillin or Ampicillin
  5. Streptococcus viridans
    DOC
    • Penicillin +- gentamicin
    • resistance may be an issue

    Alt: cephalosporin or vancomycin
  6. S. pneumoniae with PCN-susceptability of MIC<0.1mcg/ml
    Penicillin or amoxicillin

    alt: macrolide, ceph, TMP/SMX

    note: 15% of cases are highly resistant
  7. S. pneumoniae with PCN-susceptability of MIC 0.1-1mcg/ml
    DOC: penicillin G IV

    meningitis: vancomycin + ceftriaxone or cefotaxime +- rifampin
