Verapamil (Isoptin) (Calan)

  1. Description
    Used to treat narrow-complex PSVT; Second-line drug to Adenosine
  2. Mechanisim of Action
    A calcium channel blocker; Slows Av conduction and prolongs refractories in the AV node; Is effective in treating SVT; Decreases the rapid ventricular response in Atrial Tachydysrhythmias such as A-Fib and A-Flutter; Decreases myocardial oxygen demand and causes coronary and peripheral vasodilation
  3. Indications
    PSVT refractory to Adenosine; Atrial Flutter with rapid ventricular response; Atrial Fibrillation with rapid ventricular response
  4. Contrainindications
    Severe hypotension or cardiogenic shock; ventricular tachycardia; Wolff-Parkinson-White (WPW) Syndrome; a disorder of the heart where early depolarization of the heart muscle occurs
  5. Precaations
    can cause systemic hypotension; monitor BP continuously
  6. Dosage and Administration
    • For PSVT, 2.5-5mg IV over 2 minutes
    • A second dose of 5-10mg after 10-15 minutes
    • Total dose should not exceed 20mg in 30 minutes
    • No pediatric Dosing
