Increases plasmin by increasing the conversation of plasminogen to plasmin. Breaks down fibrin clots

Indications
Acute Coronary Syndrome

Contraindications
Active bleeding, suspected aortic dissection, Traumatic CPR, Severe persistent hypertension, recent head trauma, known intracranial tumor, history of stroke in 6 months, pregnancy. Benefit must out way risk if the pt has had major surgery in past 2 months, B/P 180 systolic or 110 diastolic that is controlled by medications, active ulcer disease or blood in stool, history of stroke, tumor, brain surgery, head injury, bleeding disorder, liver dysfunction, kidney failure, cancer or illness with possible thoracic abdominal or intracranial abnormalities, prolonged CPR.

- Anaphylaxis
- Emergency resuscitation drugs and equipment available
- Antidysrthythmics (amiodarone, lidocaine, diltiazem, b-blockers) available

Effects
Allergic reaction, anaphylaxis, bleeding, fever, nausea and vomiting

10 units IV over 2 min. May repeat in 30 min
