-
Interferon beta SE and MAO
- DOC, first line
- they bind to lymphocytes, causes a reduction of the migration of t cells into the cns, reduce proliferation and reduction of cytokine production, fewer macrophages stimulated and less attack on myelin and oligogendrocytes
- suppress the movement of t cells out of circ, generation of cytokines, inflamm that cytokines produce
- SE
- Suppress T cell activity--> high risk of opportunisitic infection
- flu like symptoms
- reduce wbc/rbc
- depression/suicide thoughts
- neutralizing antibodies develop (can lose its effects)
-
Avonex
- interferon- beta DMA
- treatment of MS
-
Rebif
- interferon- beta DMA
- treatment of MS
-
Betaseron
- interferon- beta, DMA
- treatment of MS
-
Glatiramer Acetate
- Copaxonetreatment of MS
- DMA
- similar to myelin based proteins (protein integrated in myelin)
- antibodies now attack this short peptide drug instead of the myelin
- raise levels of helper tcells (helper tcells suppress killer t cells from moving into the cns)
- similar to interferon
- SE
- Sub q qd
- 15mins after admin pt is chest heavy (like mi/angina)
- hard to breathe
- contraindicated in pt w <3 problems
-
Natalizumab
- Tysabri
- treatment of MS
- DMA
- infused monthly
- monoclonal antibody against alpha 4 intergins on tcells
- normally Alpha 4 beta 1 help with cell adhesion with this drug we reduce interaction between alpha beta and vcam1.. impair the interaction.. fewer adhesion/ cells moving into the blood stream
- more efficacious than interferon and copaxone 2x
- they cut clinical trails short because it was so good
- SE
- Progressive Multifocal leukoencephalopathy
- -where a virus rises up and cause encephalitis (suppressing the immune system.. increase virus and bacteria).. taken off the market but put back on bc of how good it was.. pt must seek specific drs for this and monitored carefully
- depression
-
Dalfampridine
- Ampyra
- treatment of MS
- NOT A DMA
- reduces the effects of the demyelination, neurons are still being demyelinated by tcellls
- PO
- K channel blocker, keeps K in the neuron (improves neuronal conduction)
- dont see less unmylenated, or plaques but see improvements of symps.. muscle strength/movement and walking (doesnt improve vision and speech)
- SE
- Seizures (cells more depolarized)
- UTI
-
Fingolimod
- Gilenya
- treatment of MS
- DMA and PO
- upon admin it gets phosphorylated
- agonist at the S1P1 (help tcells move out of the lymphocytes and thymus) receptor, desensitize it, now they cant respond to S1P.. Tcells wont move out of the thymus or lymph nodes.. reduction of tcells in circ
- SE
- first dose phenomenon (bradycardia.. decrease HR.. has nothing to do with the vessels like in alpha 1 antag)
- S1P is found throughout the body (smooth and cardiac muscle)
- LONG TERM SE
- hypertension, Seizures, swelling of retina, inflam bronchial, URTI
-
Mitoxantrone
- Novantrone
- treatment of seconday MS
- anticancer drug, kills killer tcells
- antineoplastic drug, not used for relapsing remitting MS
- SE
- dose related cumulative cardiotoxicity (irreversible).. based on body SA.. bc of heart size
-
Cyclosporine
- Sandimmune, Neoral, Restasis
- Immunosuppressants
- interacts w cyclophilin= inhibits calcineurin --> less dephosphorylation of nfat= less nfat in nucelus for t cell proliferation
- met by 3A4.. many DI
- ADR
- hypertention, nephrotoxic, cholesterol, hiritisum, gingival hyperplasia
