  1. To what class of drugs does sildenafil belong?
    Phosphodiesterase type 5 inhibitors (vasodilators).
  2. What are the indications for sildenafil?
    • Erectile dysfunction
    • Pulmonary arterial HTN (tadalafil)
  3. Target MOA: how does sildenafil work?
    By selectively inhibiting PDE5, it decreases the degradation of cGMP in vascular smooth muscle, allowing increased activation of MLCK and resulting dephosphorylation of myosin light chain. This allows increased blood flow to the corpus cavernosum and decreases BP in the pulmonary vasculature.
  4. What are some adverse effects associated with sildenafil?
    • Difficult blue-green discrimination in the eyes (PDE6 inhibition).
    • CNS: HA, insomnia, dizziness.
    • GI: dyspepsia.
    • CV: flushing, nasal congestion
