Aspirin
- Irreversibly inhibits COX, decreases synthesis of TXA2 and PGE
- Decreased platelet aggregation, antipyretic and analgesic, anti-inflammatory (with increasing dose)
- Can lead to acute renal failure, upper GI bleed, Reye's syndrome in kids with viral infection

NSAIDs
- Ibuprofen, naproxen, indomethicin, ketorolac
- Reversibly inhibit COX, block prostaglandin synthesis
- Antipyretic, analgesis, antiinflammatory
- Indomethicin used to close PDA
- Can cause renal damage, GI distress, ulcers

Celecoxib
- Reversibly inhibits COX2 (inflammation and pain); spares COX1 -> maintain gastric mucosa
- Rheumatoid and osteoarthritis
- Increased risk of thrombosis, sulfa allergy

Acetaminophen
- Reversibly inhibits COX, mostly in CNS
- Antipyretic and analgesic, not anti-inflammatory
- Overdose produces hepatic necrosis - glutathione depleted; N-acetylcysteine regenerated glutathione

Etidronate, pamidronate, alendronate, risedronate
- Bisphosphonates
- Inhibit osteoclastic activity
- Used in malignancy-associated hypercalcemia, Paget's disease, PMP osteoporosis
- Can cause corrosive esophagitis

Colchicine
- Used for acute gout
- binds and stabilizes tubulin to inhibit polymerization -> impairs neutrophil chemotaxis and degranulation
- Indomethicin is less toxic to GI, also used in acute gout

Probenecid
- Inhibits reabsorption of uric acid in PCT (uricosuric)
- Used for chronic gout prophylaxis

Allopurinol
- Inhibits xanthine oxidase
- Used in chronic gout, in lymphoma and leukemia to prevent tumor lysis-associated urate nephropathy
- Increases concentrations of azathioprine and 6-MP
- Do not give salicylates in gout - decreased uric acid clearance
