neonatal

  1. which babies require resuscitaion
10% of all newborns require some assistance to begin breathing
  2. ABC's are the same for adults...
    airway-
    breathing-
    circulation-
    • airway-position and clear
    • breathing-stimulate to breath
    • circulation-assess heart rate and color
  3. 100% of newborns reqiure initial assessment to determine if
    resuscitaion is required
  4. Cardio respiratory changes at birth
    pre birth
    aveoli-
    arterioles-
    blood flow-
    • filled with fluid rather than air
    • that perfuse lungs are constricted
    • diverted away from lungs through ductus arteriosis
  5. At birth after first breath (transition following birth that allows baby to get O2 into blood stream from lungs)
    Lungs-
    Blood vessels-
    Arterioles-
    Umbilical-
    • fill with air and lung fluid is absorbed from the aveoli
    • in the lung tissue relax, decreasing resistance to blood flow
    • and vein constrict and are clamped
  6. When you clamp the umbilical cord systemic blood pressure
    increases
  7. what can go wrong in transition?
    3 things
    • baby may not braethe suffciently to force fluid from aveoli
    • excessive blood loss may occur and there may be poor cardiac contractility or bradycardia from hypoxia and ischemia
    • a failure of distention of the lungs or lack of oxygen may result in sustained constriction of the pulmonay arterioles, decreasing oxygen to the lungs and body tissues
  8. The compromised child may exhibit one or more of th following symtoms 5 things
    • poor muscle tone
    • decreased respiratory drive
    • bradycardia
    • low blood pressure
    • tachypnea
  9. Is the newborn compromised?
    Primary Apnea
