MED-Morphine

    Narcotic analgesic
  2. Mechanism of Action
    Naturally occurring opiate which acts on opiate receptors in the brain providing sedation and analgesia. Also has vasodilatory properties (caused by mast cell degranulation and histamine release) which increase peripheral venous capacity and decrease venous return to the heart. Reduces cardiac workload, myocardial oxygen demand and pulmonary venous congestion.

    • CP associated with ACS
    • Pulmonary edema, CHF
    • Pain management
  4. Contraindications
    • Hypersensitivity
    • Undiagnosed head injury
    • Hypovolemia
    • Severe hypotension
    • Acute asthma
    • COPD
    • Respiratory depression
  5. Precautions
    • Administer slowly and titrate to effect
    • Undiagnosed abdominal injury
  6. Side Effects
    • Dizziness
    • N/V
    • Cramps
    • Blurred vision
    • AMS
    • Constricted pupils
    • Headache
    • Seizures
    • Drowsiness
    • Bradycardia
    • Respiratory depression
    • Syncope
    • Palpitations
    • Hypotension
    • Dry mouth
  7. Interactions
    Enhanced CNS depression with antihistamines, sedatives, barbituates, alcohol. TCAs and MAOIs may precipitate hypotension
  8. Dosage and Route
    • Adult: 1-5 mg slow IV/IM q 5 mins up to 15 mg
    • Pediatric: 0.1 mg/kg IV may repeat once after 10 mins

    Titrate to pain and systolic >100 mmhg
  9. Pharmacokinetics
    • Onset: Immediate IV
    • Duration: 2-7 hours IV
  10. Note
    Naloxone can be used to reverse effects
