  11. Where would you check for bleeding post surgery?
    Posterior to the site and anywhere that could possibly have drainage or pooling
  12. Post surgery, what is the nursing action for bleeding at the site?
    Pressure, document findings and call the DR.
  13. What is the purpose for an assesment on admission to PACU?
    Asses for complications from surgery, but most importantly the pt is at risk for cardiac and respiratory failure.
  14. What would be an example of a med error?
    Think of the 3 checks and 5 rights if any senario does not follow them that could be a med error.
  15. If there is not an order for a medication and the pt is requesting it can the nurse give it?
    NO she/he must call the DOCTOR and get an order.

  17. What is the rationale on discharge planning?
    • Continuity of care with pt, family and home health
    • Example: is pt able to take care of self? does he need home health? is the family involved in care? etc.
  18. Why would you give lasix and aldactone together?
    To balance the potassium and electrolyte levels.

    Reasoning-Lasix takes the Ka and aldactone is potassium sparing.
