MED-Dopamine

  1. Functional Class
    Sympathomimetic
  2. Mechanism of Action
    • Naturally occuring catecholamine/neurotransmitter
    • Acts on Alpha and Beta receptors (dose dependent) and dopaminergic
    • Dopamine maintains bloodflowin the renal/mesenteric vascular beds
    • Positive inotrope, increases cardiac output and systolic BP
  3. Indications
    • Cardiogenic shock
    • Hemodynamically significant hypotension (systolic 70-100 not from hypovolemia)
    • Symptomatic bradycardia
  4. Contraindications
    Pheochromocytoma, tachydysrhythmias, VF
  5. Precautions
    • In doses higher than 20 mcg/kg/min alpha effects dominate causing severe vasoconstriction
    • Monitor BP 3-5 mins
    • Can be deactivated by sodium bicarbonate
  6. Side Effects
    • Dysrhythmias
    • Palpitations
    • Tachycardia
    • Bradycardia
    • Headaches
    • Anxiety
    • N/V
    • Tissue sloughing from extravasation
    • Dyspnea
    • HTN
  7. Dose and Route
    2-20 mcg/kg/min titrate to HR

    • Dopaminergic: 2-5 mcg/kg/min
    • Beta: 5-10 mcg/kg/min
    • Alpha: 10-20 mcg/kg/min
  8. Preparation
    800 mg in 500 mL D5W or NS = 1600 mcg/mL

    400 mg in 250 mL D5W or NS = 1600 mcg/mL
  9. Note
    Should not be used as the sole agent in hypovolemic shock. Fluid resuscitation is primary treatment
