  1. Cetirizine (Zyrtec)
    • Second generation
    • Class: Piperazine peripheral selective

    Indication: allergic rhinitis and chronic idiopathic urticaria

    MOA: selective agents that bind only peripheral receptors and as a group are less sedating

    Racemic mix: 10 mg qd

    Dose needs to be reduced for renal and hepatic failure
  2. Levocetirizine (Xyzal)
    • Second generation
    • Class: piperazine peripheral selective

    • One enantiomer
    • Dose= adults 5 mg qd=dose is cut in half

    Indications: allergic rhinitis and chronic idiopathic urticaria 

    MOA: selective agents that bind only peripheral receptors and as a group are less sedating

    approved for use in children 6-11 years: get one half 5mg tablet
  3. Loratadine (Claritin or Reditabs)
    • Second generation
    • Class: Piperidine peripheral selective 

    Indication: Allergic rhinitis and urticaria

    MOA: selective agents that bind only peripheral receptors and as a group are less sedating
  4. Fexofenadine HCl (Allerga)
    • second generation
    • Class: Piperidine peripheral selective

    Indications: seasonal allergic rhinitis (children 6 years of age or older) and chronic idiopathic urticaria

    MOA: selective agents that bind only peripheral receptors and as a group are less sedating

    less sedation at higher doses than other 2nd generation drugs

    taking with fruit juice can decrease absorption by up to 70% by inhibiting OATP1A2 transporters in the Intestinal tract.
  5. Olopatadine HCl (Patanase)
    Nasal spray
    New molecular entity

    Indication: prevention of itching associated with seasonal allergic rhinitis

    MOA: Relatively selective dual H1-receptor antagonist AND a inhibitor of histamine and other mediator release from mast cells by acting as a mast cell stabilizer--DUAL MECHANISM

    No alpha-adrenergic, dopaminergic or muscurininc type 1/2 receptor effects
