  1. Dornase Alfa (Pulmozyme)
    Mucolytic Respiratory Products

    Indication: Cystic Fibrosis

    • MOA:This enzyme is delivered by inhalation to assist in the clearance of bronchial secretions due to the inflammatory processes and death of leukocytes, bacteria and tissue large amount of extracellular DNA accumulate.
    • The buildup of DNA leads to increased viscosity of sputum. This enzyme has NO EFFECT on intracellular DNA
    • The enzyme cleaves the DNA decreasing viscosity improving mucociliary clearance of sputum decreasing recurrent bacterial infections

    Do NOT use the same neubulizer for other inhalation medications

    Contraindicated: breast feeding, children under 5, and hamster protein sensitivity

    Adverse effects-chest pain, cough, dyspnea- disease or actual side effect

    must be stored in refrigerator
  2. Acetylcysteine
    Mucolytic Respiratory Product

    • Indications: adjunctive therapy for abnormal, vicid, or inspissated mucus secretions in chronic bronchopulmonary disease: COPD, asthma, CF, posttraumatic chest conditions, anesthesia, tracheostomy care. 
    • Antidote to hepatotoxins including acetaminophen overdosage

    MOA: thiol group disrupts the disulfide linkages in mucoproteins causing depolymerization and a drop in viscosity. NO affect on DNA
  3. Aztreonam (Cayston)
    Inhaled Antibiotic

    Indication: treatment of CF patients to improve pulmonary function when they are infected with Pseudomonas aeruginosa. First inhaled antibiotic and useful in resistant strains using specific nebulizer product Altera

    MOA: Monobactam-inhibits cell wall crosslinking
  4. Tobramycin Sulfate
    (TOBI)-inhalation powder used in CF to control Pseudomonas aeruginosa

    (Bethkis)-inhalation solution for nebulizer used in CF
    inhaled antibiotic
