Fluid Therapy

  1. What is the fluid deficit equation?
    FWD = ([(Current Na+) ÷ (Normal Na+)] – 1) x (0.6 x BW kg)
  2. Causes of Pure Water Deficits:
    • Primary hypodipsia
    • Diabetes Insipidus
    • Heat stroke
    • Fever
    • Inadequate access to water
  3. Causes of Hypotonic Fluid Losses:
    • GIT: V, D, SI obstruction
    • Third spacing
    • Burns
    • Renal: osmotic diuresis (mannitol, DM), chemical diuretics, renal failure (acute and chronic), post obstructive diuresis
  4. What are the physiologic responses to increased osmolality
    • ADH release: activates V2 receptors on renal collecting tubular cells, aquaporin-2 molecules inserted into luminal membrane, water retention via aquaporins
    • Thirst
