  2. what is the spinal canal in the cervical region?
    • large
    • risk of injury (swelling) is reduced with immobilisation
  3. what is the spinal canal in the thoracic region?
    • wider and the spinal column is narrower 
    • injury is more likely to completely disrupt and damage the spinal cord
  4. what is neurogenic shock?
    • state of poor tissue perfusion caused by sympathetic tone loss 
    • often after spinal cord injury
  5. what leads to hypotension in neurogenic shock?
    an interference with normal nervous control of blood vessel diameter
  6. what are some risk factors for spinal cord injuries/ neurogenic shock?
    • sporting injuries 
    • falls
    • RTCs
  7. what are the signs of a spinal cord injury?
    • neck or back pain (central bony tenderness) 
    • reduction/ loss of sensation or movement in limbs 
    • burning sensation in trunk or limbs 
    • sensation of electric shock in trunk or limbs
  8. symptoms of spinal cord injuries?
    • diaphragmatic or abdominal breathing 
    • warm peripheries/ vasodilation in presence of hypotension 
    • flaccid muscles with absent reflexes 
    • Priapism- full/ partial sustained erection of the penis
  9. what are the signs of neurogenic shock?
    • hypotension (systolic BP often 80-90 mmHg)
    • bradycardia
  10. how would you treat spinal cord injury/ neurogenic shock?
    • request para/ hems 
    • 100% o2 using non re-breather mask 
    • keep movement to a minimum 
    • use a blanket- prevent hypothermia 
    • immobilise patient 
    • use KED/ rescue board for extrication
    • do not transport patient on long board- use orthopaedic stretcher
  11. on what stretcher should a patient be transported to hospital?
    orthopaedic stretcher
  12. what should happen once the patient is packaged and ready to go?
    • time critical, minimal time on scene- smooth journey 
    • pre alert to MTC via PD09
    • constant reassessment, if possible use end tidal capnography to monitor respirations en route 
    • only do 2nd survey en route to hospital if time permits
