    Documents the destruction of controlled substances
  2. DEA 106
    Used to report lost or stolen controlled substances when the loss is >5% of the yearly product that is stolen or missing
  3. DEA 222
    A triplicate form that requires 3 copies for ordering and returning CII drugs
  4. DEA 222 blue copy
    Pharmacy (purchaser) proof of purchase keeps blue copy on file for two years
  5. DEA 222 green copy
    The seller sends the green copy to the local DEA (the pharmacy becomes the seller, and the entity receiving the returned outdated CII drugs becomes the purchaser)
  6. DEA 222 brown copy
    The seller keeps the brown copy on file.  The seller can also be the pharmacy that is returning any controlled medication that is outdated
  7. DEA 224
    Required for a pharmacy to dispense controlled substances
  8. DEA 225
    Required to manufacture or distribute controlled substances
  9. DEA 363
    Required for compounding controlled substances or to operate treatment programs requiring controlled substances
