    • analgesics with CNS effects,
    • antipsychotics,
    • some antidepressants: mirtazapine (Remeron),
    • trazodone (Desyrel),
    • dopamine agonists (ropinirole, etc.),
    • antihistamines,
    • antinauseants,
    • anticonvulsants,
    • muscle relaxants,
    • antihypertensives with CNS effects,
    • CII-V (2-5) depressant or narcotic drugs including hypnotics-controlled and non-controlled
  9. May cause blurred vision
    • amiodarone,
    • anticholinergics,
    • ethambutol,
    • hydroxychloroquine,
    • isoniazid,
    • isotretinoin,
    • PDE 5 inhibitors (sildenafil, others),
    • scopolamine patch,
    • tamoxifen,
    • telithromycin,

    toxic levels: digoxin yellow halos, alcohol, CNS depressants
  10. Do not use alcohol while taking this medicine
    • disulfiram,
    • tinidazole, metronidazole (avoid 48 hours after last dose),
    • nitrates,
    • opioids (all, special warning for Opana),
    • tramadol,
    • tapentadol (Nucynta),
    • benzodiazepines,
    • barbiturates,
    • non-benzodiazepines hypnotics (zolpidem, etc.),
    • anticonvulsants,
    • antipsychotics,
    • some antidepressants,
    • skeletal muscle relaxants,
    • insulin,
    • metformin,
    • sulfonylureas
  11. Avoid prolonged exposure to direct and/or artificial sunlight
    • sulfa antibiotics,
    • fluoroquinolones,
    • tetracyclines,
    • topical retinoids (newer ones less risk),
    • isotretinoin (oral),
    • ritonavir + few other HIV drugs,
    • NSAIDs (piroxicam, diclofenac, some risk with ibuprofen, naproxen),
    • metronidazole,
    • isoniazid,
    • diuretics
  12. Take on an empty stomach
    • ampicillin,
    • efavirenz (Sustiva/Atripla - preferably at bedtime),
    • bisphosphonates (30 mins + before breakfast, 60 mins + for Boniva),
    • captopril (1 hour before meals),
    • didanosine (Videx), 
    • indinavir (Crixivan),
    • iron (if tolerated),
    • PPIs (variable times, all before eating),
    • tadalafil and sildenafil (light meal, avoid fatty food),
    • levothyroxine (30-60 mins before breakfast or 3-4 hours after last meal),
    • Opana,
    • mycophenolate (CellCept), 
    • tacrolimus extended release (Astagraf XL, Envarsus XR), 
    • voriconazole (VFEND),
    • zafirlukast
  13. Take with food
    • atazanavir (Reyataz),
    • carvedilol (Coreg),
    • itraconazole capsules,
    • metformin (IR with breakfast and dinner, XR with dinner),
    • lovastatin (with dinner),
    • fenofibrate and derivatives (Lofibra, Lipofen, Fenoglide),
    • niacin, 
    • gemfibrozil (Lopid - 30 min before breakfast and dinner),
    • phosphate binders (when eating),
    • NSAIDs,
    • opioids (except Opana),
    • steroids,
    • metoprolol tartrate (Lopressor), lithium
  14. Take with full glass of water
    • sulfamethoxazole/trimethoprim (Bactrim),
    • bisphosphonates,
    • sulfasalazine (Azulfidine + food)
  15. Do not eat/drink grapefruit/juice at any time
    • lovastatin,

  17. May cause discoloration of the urine, skin, and sweat. May stain contact lenses and clothing.

  18. Check for peanut or soy allergy

  19. Separate from dairy products, calcium, magnesium or iron tablets, antacids
    • tetracyclines,
    • quinolones
  20. Do not chew or crush, swallow whole
    • enteric coated (bisacodyl, etc),
    • any drug that ends with XR, ER, LA, SR, CR, CRT, SA, TR, TD,
    • or has 24 in the name,
    • or the ending "-cont" (for controlled release, such as MS Contin or Oxycontin), 
    • timecaps,
    • sprinkles
    • metoprolol XL and levodopa-carbidopa cut at score line (for half the dose), but cannot crush or chew
  21. Persistent diarrhea - due to colitis may occur weeks after using this medication - report to your doctor immediately if this occurs
    • clindamycin,
    • broad-spectrum antibiotics
  22. Warning: This medicine may make birth control less effective
    • barbiturates (phenobarbital, etc.),
    • St. Johns wort,
    • some HIV drugs (some of the protease inhibitors, NNRTIs),
    • ampicillin,
    • tetracycline,
    • rifampin,
    • rifapentine,
    • griseofulvin,
    • anticonvulsants (topiramate, lamotrigine, carbamazepine, primidone, phenytoin, oxcarbazepine),
