Diuretics and HF

  1. HCTZ
    • hydrouiril
    • Thiazide--> increases NA and CL excreted from distal tubule
    • Dose: 12.5- 100mg qd
    • DI: digoxin, corticosteroids, NSAIDs,
    • SE: photosensitivity, sexual dysfunction, sulfa

  2. Furosemide
    • Lasix
    • Loop--> blocks absorption of sodium and cl in proximal tubule
    • DOSE: 20-80mg qd
    • DI: corticosteroids, aminoglycosides, NSAIDS,
    • SE: hypouricemia, hypokalemia, sulfa
  3. Sprinolactone
    • aldactone
    • aldosterone antagonist: competes with aldosterone at the na/k exchange site of the distal tubule --> inc K retention and increases na excretion
    • dose: 25-200 mg in divided doses
    • DI: ACE , NSAIDs, K
    • SE: hyperkalemia, gynecomastia, ha, nvd
  4. Triamterene/ hctz
    • dyazide/maxide
    • Potassium sparring + thiazide
    • dose: 37.5-75/ 25-50 mg qd
    • DI: ace, NSAIDs,
    • SE: hyperkalemia, gynecomastia, ha, nvd, digoxen, corticosteroids, photosensitivty, sexual dysfunction, sulfa
