    • No calculus or defective margins are detected.
    • Bleeding is evident with gentle probing.
  6. PSR Code 2
    • Colored band is completely visible in the deepest sulcus or pocket in the sextant.
    • Supra- or subgingival rough surface or calculus/and/or defective restoration margins are detected.
  7. PSR Code 3
    • Colored band is partially visible in the deepest sulcus or pocket in the sextant.
    • Calculus, defective restorations, and bleeding may or may not be present
  8. PSR Code 4
    • Colored band is not visible in the deepest sulcus or pocket in the sextant.
    • Indicates a probing depth of 5.5 mm or greater.
    • No need to continue probing sextant once a 4 is obtained (Codes 0-3: probe entire sextant)
  9. The symbol (*) is added to any sextant exhibiting any of the following abnormalities:
    • Furcation involvement
    • Mobility
    • Mucogingival defects
    • Recession extending into the colored band
