  2. What is the potassium infusion limits?
    10 mEq in 100 mL over 1 hour
  3. Name some indications for IV Therapy?
    • inability to eat and admin tube feedings
    • dehydration
    • electrolyte imbalance
    • long-term med therapy
    • emergency med admin
    • blood admin
  4. How long do we leave in short peripheral catheters?
    • 96 hours (mostly in San Diego)
    • 72-96 hours - then remove and new insertion site
  5. What is the desired gauge catheter for Med/Surg?
    • 20 gauge
    • 20-24 is acceptable - depending on the pt
  6. How many sticks are done before getting someone else to insert IV?
    2
  7. how many seconds do we scrub the skin with chlorhexidine solution?
    30 seconds, then allow to dry for 30 seconds
  8. How many seconds to we scrub the hub before injecting into IV?
    • 15 seconds
    • done before saline, before med, before saline (EVERY TIME)
  9. How often do we assess peripheral IV site?
    • Every shift
    • Q2 hours if running IVF
  10. How often must we change IV fluid bags?
    • prior to running dry
    • every 24 hours whether empty or not
  11. How often must we change IV tubing (primary & secondary)?
    every 96 hours
  12. How often must we change blood tubing?
    within 4 hours
  13. Leakage of a non-vesicant solution into tissue?
    infiltration
  14. Leakage of a vesicant solution into tissue?
    extravasation
  15. What do you do if the IV site infiltrates or extravasates, or you suspect phlebitis?
    • Stop infusion immediately
    • Remove IV (may send to lab for culture - if phlebitis)
    • Inform provider - may call pharmacy for antidote drug (if exists)
    • start new IV
  16. Name the phlebitis scale grades:
    • 0 - No symptoms
    • 1 - Erythema at access site with or without pain
    • 2 - Pain at access site with erythema or edema
    • 3 - Pain at access site with erythema or edema streak formation, palpable venous cord
    • 4 - pain at access site with erythema and/or edema, streak formation, palpable venous cord >1" in length, purulent drainage
  17. What are S/S of speed shock?
    • dizziness
    • irregular pulse
    • flushing
  18. What is speed shock and how can we avoid it?
    • Pushing or infusing IV med too quickly
    • Check to see how fast med should be administered
    • Check to see if med should be diluted
  19. How do we prime IVPB (IV piggy back) tubing?
    • back primed
    • lower bag below primary and open tubing
  20. what should we do prior to adding a secondary med to primary infusion?
    • Check compatibility of secondary to primary
    • research administration rate
  21. What is the first thing to do when opening a package of tubing?
    CLOSE clamp
