Create
Inotropic Drugs
Sympathomemetics
Enhance
Mycocardial contractility
by stimulating adrenergic receptors
Special Considerations:
Continuous monitoring of ECG, BP, HR, peripheral circulation
Inotropic Drugs
Sympathomemetics
Dopamine (Intropin)
Dobutamine (Dobutrex)
Epinephrine (Adrenaline)
Norepinephrine (Levophed)
Isoproterinol (Isuprel)
Dopamine (Inotropin)
Inotropic Drug
Sympathomemetic
3mcg/kg/min - used for
kidney perfusion
3-10 mcg dose
- B1 stimulation:
improves heart contractility and CO
>10mcg/kg/min-Alpha stimulation -
vasoconstriction
Dobutamine (Dobutrex)
Inotropic Drug
Sympathomemtic
Affects
B1 receptors
of heart -
increased contractility without increases in BP
from vasoconstriction
Epinephrine (Adreniline)
Inotropic Drug
Sympathomemetics
Low doses stimulate Beta receptors
:
Increase HR, Contractility, vasodilation
High doses (>2mg/min) stimulate alpha receptors
Vasoconstricion
Norepinephrine (Levophed)
Inotropic Drug
Sympathomemetics
Stimulates
Beta 1 (heart)
and
Higher doses stimulate Alpha receptors (vascular)
Isoproterinol (Isuprel)
Inotropic Drug
Sympathomemetic
NEVER GIVE IV PUSH - only give IV Drip
Pure
Beta receptor stimulant
with no alpha effects
Limited usefulness do to dysrhythmia issues
Profound increases in HR, Conduction, and Contractility.
Produces vasodilation of pulm arteries and bronchodilation.
