Cardio Pharmacology

  1. Inotropic Drugs
    Sympathomemetics
Enhance Mycocardial contractility by stimulating adrenergic receptors

    Special Considerations:

Continuous monitoring of ECG, BP, HR, peripheral circulation
  2. Inotropic Drugs
    Sympathomemetics
    • Dopamine (Intropin)
    • Dobutamine (Dobutrex)
    • Epinephrine (Adrenaline)
    • Norepinephrine (Levophed)
    • Isoproterinol (Isuprel)
  3. Dopamine (Inotropin)
    • Inotropic Drug
    • Sympathomemetic

3mcg/kg/min - used for kidney perfusion

3-10 mcg dose - B1 stimulation: improves heart contractility and CO

    >10mcg/kg/min-Alpha stimulation - vasoconstriction
  4. Dobutamine (Dobutrex)
    • Inotropic Drug
    • Sympathomemtic

Affects B1 receptors of heart - increased contractility without increases in BP from vasoconstriction
  5. Epinephrine (Adreniline)
    • Inotropic Drug
    • Sympathomemetics

    • Low doses stimulate Beta receptors:
    • Increase HR, Contractility, vasodilation

    • High doses (>2mg/min) stimulate alpha receptors
    • Vasoconstricion
  6. Norepinephrine (Levophed)
    • Inotropic Drug
    • Sympathomemetics

    • Stimulates Beta 1 (heart) and
    • Higher doses stimulate Alpha receptors (vascular)
  7. Isoproterinol (Isuprel)
    • Inotropic Drug
    • Sympathomemetic

NEVER GIVE IV PUSH - only give IV Drip

Pure Beta receptor stimulant with no alpha effects

    • Limited usefulness do to dysrhythmia issues
    • Profound increases in HR, Conduction, and Contractility.
    • Produces vasodilation of pulm arteries and bronchodilation.
