1750: Thyroid disease

  1. What hormones do the thyroid produce?

    How do the hormones effect metabolism?

    T3/T4 needs iodine to synthesize.
    T3: strong; speeds up metabolism.

    T4: weak; maintains steady metabolism.

    calcitonIN: decrease serum Ca++ levels by pulling Ca++ and depositing it into bone.
  2. Labs & Diagnostics.

    What does the Pituitary gland secrete when thyroid hormone is low?
    • TSH: (in pituitary) stimulates the secretion of thyroid hormone.
    • Serum T3/T4
    • Thyroid Abs
    • Thyroid Scan
    • Biopsy
    • *RAI (radioactive Iodine Uptake): how good the thyroid is taking up the iodine.
  3. What should you assess prior to labs & diagnostics?
    Does the Pt have allergies to shellfish or iodized salt?

    When was last intake of iodine?
  4. Hypothyroidism.

    What causes the thyroid gland to enlarge?

    What is "myxedema" (a result of hypothyroidism) and what are the s/sx?
    • Thyroid gland enlarges in attempt
    • to produce more TH.

    Older clients have decrease in T4.

    Puffy face, large tongue, husky voice.
  5. What are some RF to hypothyroidism?
    Women over 50

    Radiation to neck

    Iodine deficiency

    *hashimoto's thyroiditis (autoimmune)
  6. What is the first line of drug for hypothyroidism?
    Levothyroxine.
  7. Congenital Hypothyroidism

    (cretinism) What are s/sx (characteristics) of this congenital disorder?
    Physical deformity

    MR

    Dwarfism

    *Goiter
  8. Congenital hypothyroidism.

    What are some PCs of cretinism?
    Intellectual disability if untreated

    Short stature

    Delayed physical growth
  9. What is the drug of choice for cretinism?

    What are some nursing interventions?
    L-thyroxine (levothyroxine)

    Monitor & record growth

    Monitor labs

    What is it caused by?

    What can it cause regarding metabolism?
    Grave's disease.

    Excess secretion of TH.

    Increases metabolism.

    What would you see in this disease?
    Increased appetite w/ weight loss.

    Goiter

    Hypermotile bowels

    Heat intolerance, insomnia

    Palpitations

    Increased sweating

    Hair changes

    Nervousness/anxiety
