  1. Heparin is a
    • Anticoagulant- Unfractionated heparin
    • Subq or IV
  2. Monitor Heparin ?
    • PTT ***only for IV** Not subq
    • INR
    • Platlets

    25-35 secs (heparin)
  4. Normal INR
    2.0 to 3.0 (warfarin)
  5. Normal PT
    10.0 to 14.0
  6. Normal Platelet count
    140 to 440
  7. Warfarin (coumadin)
    • A vitamin K antagonist
    • indicated for extended anticoagulant therapy
    • monitor INR
    • delay effect 3 to 5 days (slow onset)
    • reduce effect with food containing Vitamin K

    Vitamin K (phytonadione)
  9. the antagonist to Heparin
    protamine sulfate IV

    To stop bleeding
  10. low-molecular-weight heparin (LMWH)
    • dalteparin (Fragmin)
    • enoxaparin (Lovenox)

    • Doesn't Get Monitored
    • safer for Pregnant
  11. Treatment for haparin induce thrombocytopenia
    • lepirudin (Refludan)
    • argatroban (Novastan)
  12. prophylaxis during major surgery (hip or knee)
    • fondaparinux (Arixtra)
    • Subq - *NOT for IM use
    • Don't use with LMWH or Heparin
    • Can be use with Warfarin
    • No monitory
    • via unchanged through Kidney (caution with Patient with renal Insufficiency)
    • anticoagulant

  14. Thrombolytic therepy
    • "clot Buster" -"ASE"
    • alteplase (Activase, t-PA)

    • tenectplase

    • Given first 3 days of acute thrombosis
    • beyond 14 days less effective
  15. Antiplatelet
    • Aspirin
    • Clopidogrel (Plavix)

    prevent formation of platelet
  16. Antifibrinolytic: prevent lysis of fibrin
    • Amicar - aminocaproic acid
    • tranexamic acid
    • Desmopressin (may be use with Hemophilia)

    hemophiliacs take to promote clotting
  17. Meds for PAD
    • Pentoxifylline & cilostazol (for symptomatic cluadication)
    • Antiplatelets
  18. Med for abdominal Aortic Aneurysm
    • Antihypertensive
    • Diuretics
    • Betal Blockers
    • Ace-inhibitors
    • Angiotensin II antagonist
    • Calcium channel Blocker
  19. ulcers
    • providone iodine
    • cadexomer iodine
    • acetic acid
    • chlorhexidine
