- Do not exceed 140 mg/m2

- Readily crosses blood brain barrier
- CNS levels = plasma levels
- Stem cell transplant/bone marrow transplant
- Seizures –Prophylax with phenytoin (Keppra also used) for 48-72 hrs after dose completed
- IV and PO = not interchangeable
