    • Std dose: Myelosupression
    • >1gm/m2: Cerebellar syndrome (DL, HT), conjunctivitis

    Neuro checks before each dose for Cerebellar syndrome (slurred speech, ataxia, confusion)
  10. What are the Common uses for Azacitidine?
    • MDS
    • CML
  11. What are the SE for Azacitidine?
    • Myelosupression
    • Febrile neutropenia
    • Fever
  12. What special information should you know about Azacitidine?
    • No live vaccines
    • <5% cardiac problems (A fib and CHF)
    • SQ injection

    • Hyperglycemia (HT)
    • Edema
    • Pneumonia

    • No live vaccines
    • <5% cardiac toxicity (A fib or CHF)

    • NSCLC and Metastatic Breast cancer
    • Increased QOL – DOC – Pancreatic cancer

    All, NHL, breast cancer, Osteosarcoma
  18. What are the SE for Methotrexate?
    • LD: Hepatotoxicity and pulmonary toxicity
    • HD: Myelosupression, Mucositis and Diarrhea (HT and DL), Nephrotoxicity (HT)
  19. What special information should you know about Methotrexate?
    • DDI with Bactrim and NSAIDs
    • Hyrdrate w/ Bicarb, keep urine >7 pH until drug levels are safe
    • Rescue: give leucovorin 24-36hr post dose with doses >100 mg/m2, use until levels drop below 0.05 or 0.02

    • B12 100 mcg IM before and Q9wk
    • Dexamethasone 4 mg BID x3 days, starting before therapy

    • PN (DLT)
    • Hypersensitivity
    • Myalgias
  38. What special information should you know about Ixabepilone?
    • CYP3A4 metabolized
    • No PVC
    • May cause cardiac dysfunction
    • Pre-medicate with Benadryl and H2 antagonist (Dexa if hypersensitivity occurs)
    • CI in hepatotoxicity
