Progresses from restlessness and confusion to decreasing level of consciousness, unresponsiveness, and coma
Altered Vital Signs
Examine for:
Increased BP
Widening pulse pressure and slowing of pulse
Irregular respirations including periods of apnea
Cheyne-Stokes respirations
Elevated temperature
Vomiting
Secondary to irritation of vagal nuclei, CN X
Pupillary Changes
CN III signs
Examine for:
Ipsilateral dilation of pupil (unequal pupils)
Slowed reaction to light progressing to fixed
Dilated pupils (a poor prognostic sign)
Progressive Impairment of Motor Function
Examine for:
Weakness
Hemiplegia
Positive Babinski response
Decorticate or decerebrate rigidity
