  1. Colostomy assessment.

    When inspecting the patient's stoma, what do you want to note?
    Size (stabilizes within 6-8 weeks).

    Keep the skin around the stoma site clean and dry.

    What can you teach to the patient about colostomy care?

    Encourage and avoid?
    *Keep patient as free of odors as possible.

    Empty appliance frequently.

    • Encourage:
    • odor-free
    • clean and dry stoma site
    • drink 2 qts (or L) of water daily
    • diet and exercise
    • Resume normal activity (work, sex, etc.)

    • Avoid:
    • foods high in fiber
    • foods that cause diarrhea or flatus
  3. What are some "comfort measures" or nursing actions?
    Use meds as needed.

    Apply ointments or astringent (witch hazel) to decrease itching and skin irritation/inflammation.

    Use suppositories that contain anesthetics.
