• CLASS: Narcotic (opioid) antagonist

• Prevents or Reverses the effects of opioids including: 
• Respiratory depression
• Sedation
• Hypotension

Antidote for: Opioid overdoses

• Conversion of SVT with no known atrial fibrillation or atrial flutter.
• Wide complex tachycardia of uncertain origin unresponsive to lidocaine

• Slows conduction through AV node
• Slows sinus rate
• Larger doses decrease BP by decreasing peripheral resistance

• 6 mg rapid IV bolus 
• may repeat 12 mg twice

• 2nd or 3rd degree AV blocks

• CLASS: Antiarrhythmic, local anesthetic

Suppression of ventricular arrhythmias (VT,VF, PVC's)

Prophylaxis against recurrence after conversion from VT or VF

Frequent PVC's (> 6 per min; 2 or more in a row; multiform PVC's; or R-on-T phenomenon).

• Pulseless VF/VT: Initial bolus of 1.0-1.5 mg/kg IV PUSH every 3-5 minutes to a total of
• 3 mg/kg.

• Antidysrhythmic or rhythms with a pulse: Initial boluses IV 1.0-1.5mg/kg 
• Additional boluses 0.5-0.75 mg/kg every 5-10
• min. to a total dose of 3 mg/kg.

Maintenance Infusion:  After ROSC 2gm 500 ml and initiate a drip at 2-4 mg/min 

• Patients > 70 years or with hepatic, renal disease or poor perfusion state, reduce
• maintenance infusion by half.

• 15 sec. = 1mg
• 30 sec. = 2mg (1drip every 2 sec.)
• 45 sec. = 3mg
• 60 sec. = 4mg (1drip every sec.)
