    Initial Care
    • Initial medical care 
    • Obtain 12 lead
    • Establish vascular access
    • IV success? yes/no
  2. ACS / Chest Pain Adult >15
    If IV is successful
    and
    Patient is hypotensive (systolic <90)
    • Give one fluid challenge of 250mL
    • (ensure lung sounds are clear)
  3. ACS / Chest Pain Adult >15
    YES: IV is successful
    Drug administrative
    • Aspirin
    • 2-4 Baby aspirin (81mg) by mouth
    • (no allergy)
    • Nitroglycerin 
    • 0.4 mg SL tablet or spray (x3 every 5 mins)
    • systolic >90
    • Morphine
    • IV: 2-4 mg increments up to 10mg 
    • BP >90
    • or
    • Fentanyl
    • IV: 50-100 mcg (repeat to max of 200 mcg)
    • IM: 2mcg/kg (max.of 200mg)
    • Intranasal: 2mcg/kg (max.of 200mg)
