  1. Define Frank Breech
    Feet up! Bent at hips, but down
  2. Define Complete Breech
    All joints flexed - hips bent, knees bent
  3. Maternal/Fetal complications of breech birth
    • 1. involuntary pushing and potential entrapment with presenting part coming through cervix <10cm
    • 2. arrest of head
    • 3. early ROM
    • 4. cord prolapse
    • 5. cord compression
    • 6. potential aspiration - shock of cold on body before head out
    • 7. head subject to rapid de-/compression
    • 8. maneuvers may cause injury (palsy etc.)
  4. Guidelines for breech birth
    • 1. term, uncomplicated fetus
    • 2. EFW >2500 g <4000g
    • 3. frank/complete
    • 4. head flexed & normal size
    • 5. adequate pelvis
    • 6. spontaneous labor with normal progress
    • 7. adequate AF
  5. Labor guidelines for breech
    • - maintain intact membranes
    • - expect steady progress
    • - assure full dilation
    • - enable mother to arrive physically & mentally
    • - empty bladder
    • - AVOID HANDS & KNEES
    • - HANDS OFF presenting part!
    • - do NOT SPEED birth
    • - umbilicus to airway <5min.
