Anatomy 1 Lecture 13 - Gluteal Region

  1. What are the boundries of the gluteal region?
    • Superior: Ilaic crest
    • Medial: Sacrum and Coccyx
    • Inferior: sacrotuberous and iscial tuberosity
    • Lateral: Greater trochanter
  2. Explain the cutaneous innervation of the gluteal region.
    • Superior cluneals (dorsal rami first 3 lumbar nerves)
    • Middle cluneals (dorsal rami of first 3 sacral nerves)
    • Inferior cluneals (posterior femoral cutaneous which is a branch of the sacral plexus which comes off the primary ventral rami)
    • branches of subcostal, iliohypogastric and lateral femoral cutaneous may also reach the area.

  3. What is the primary motion of the muscles of the gluteal region?
Abduction and lateral rotation of the thigh
  4. What does the sacrotuberous ligament do? Where does it run?
    • It prevents nutation (anterior tipping of the sacrum)
    • From the posterior inferior iliac spine, sacrum and coccyx to the ischila tuberosity
  5. Give me the details about the sacrospinous ligament.
It extends from the sacrum and coccyx to the ischial spine. It is anterior to the sacotuberous ligament.
  6. What is one of the functions of the sacrotuberous and sacrospinous ligaments?
They help to turn the sciatic notches into the sciatic foramina.
  7. Gluteus Maximus OINA
    • O: Ilium posterior to the posterior gluteal line, sacrum and sacrotuberous ligaments
    • I: Gluteal tuberosity of the femur and the iliotibial band.
    • N: inferior gluteal n. (comes out below the piriformis in the greater sciatic notch)
    • A: Extends teh thigh when running and climbing, also Abducts and Ex. Rotates the thigh
  8. Piriformis OINA
    • O: Pelvic surface of the sacrum (inside)
    • I: greater trochanter
    • N: Nerve to piriformis
    • A: Lateral rotation and extension of thigh (there is some debate as to what it really does)
  9. Gluteus Medius OINA
    • O: Ilium between iliac crest and superior gluteal line
    • I: greater trochanter
    • N: Superior gluteal n.
    • A: Abduct and internally rotates the thigh
  10. Obturator Internus OINA
    • O: Obturator membrane
    • I: Greater trochanter
    • N: n. to obturator internus
    • A: Laterally rotates and extends thigh
  11. Superior gemellus OINA
    • O: Ischial Spine
    • I: Greater Trochanter
    • N: n. to obturator internus
    • A: Laterally rotates and extends thigh
  12. Inferior Gemellus OINA
    • O: Ischial tuberosity
    • I: greater trochanter
    • N: n. to quadratus femoris
    • A: laterally rotates and extends thigh
  13. Quadratus Femoris OINA
    • O: Ischial tuberosity
    • I: intertrochanteric crest
    • N: n. to quadratus femoris
    • A: Laterally rotates and extends thigh
  14. Gluteus Minimus OINA
    • O: Ilium between the superior and inferior gluteal lines
    • I: Greater trochanter
    • N: superior gulteal
    • A: Abduct and medially rotate the thigh
  15. Obturator Externus OINA
    • O: obturator membrane
    • I: Trochanteric fossa
    • N: Obturator n.
    • A: Adduct and laterally rotate the thigh
  16. Tensor Fascia Lata OINA
    • O: Iliac crest
    • I: Iliotibial band
    • N: Superior gluteal n.
    • A: Abducts and flexes thigh
    • (tension in the IT band)
  17. Where do the superior gluteal n and vessels pass? Which muscle do the vessels supply?
    • Through the greater sciatic foramen
    • The glut max, med, mini, and TFL
  18. Where does the inferior gluteal nerve and vesselrun? What do they supply?
Exit through the greater sciatic foramen to the piriformis and enter deep surface of the gluteal maximus. The glut max is inervated by the inferior, and not the superior gluteal. The superior gluteal innervates the med, min and TFL.
  19. Where does the sciatic n. and vessels pass? What do they supply?
Through the greater sciatic notch. They supply the muscles of the posterior compartment
  20. Where does the posterior femoral cutaneous n. pass? what does it supply?
Runs medial to the sciatic n. Supplies cutaneous information to the posterior thigh.
  21. Where does the nerve to the obturator internus pass? What does it supply?
    • Exits the creater sciatic foramen and passes immediately into the lesser sciatic foramen.
    • Supplies motor innervation to the obturator internus nad superior gemellus.
  22. Where do the pudendal n. and vessels pass? What do they supply?
They exit through the greater sciatic foramen and pass immediately into the lesser sciatic foramen just medial to the nerve to the obturator internus
  23. Where does the n. to the quadratus pass? what does it supply?
Exits the greater sciatic foramen inferior to the piriformis and descends anterior to the obturator internus and gemelli muscles. Innervates the inferior gemellus and quadratus femoris muscles.
