Lidocaine Actions
- 1. Suppress ventricular ectopy
- 2. Increase VF threshold
- 3. Reduce electrical impulse through heart

Lidocaine Indications
1. Malignant PVC's with suspected ischemia (a: greater than 6 per min B: myocardial infarction C: couplets / salvos or runs of VT D: R-on-T pehnomenon E: multifocal F: symptomatic)
2. VT, VF
3. Wide-Complex Tachycardias with unknown origen

Lidocaine Contraindications
- 1. High degree heart block (2nd and 3rd)
- 2. PVS's with bradycardia
- 3. Allergy to any 'caine' medication
- 4. idioventricular rhythms
- 5. Do not give with beta blockers or dopamine

Lidocaine Precautions
- 1. dosage not to exceed 300mg / hr
- 2. monitor for CNS toxicity
- 3. reduce dose by 50% in patients over 70 years of age or who have liver disease
- 4. Use bolus therapy in cardiac arrest only

Lidocaine Side Effects
- 1. anxiety, drowsiness, dizziness, confusion
- 2. nausea, vomitting, convulsions
- 3. widening of QRS
- 4. hypotension, numbness, slurred speech
- 5. parasthesias

Lidocaine Dosage
- PVS's, VF, Wide Complex Tachycardia:
- 1.0-1.5 mg / kg repeat every 5 to 10 minutes to a max of 3.0 mg / kg
- Drip
- 2-4 mg / min - 1 gram in 250 ml

Lidocaine Route
ET (ET max dose is 6 mg / kg total when all doses are delivered ET), IV bolus, IV infusion, IM Bolus of 300mg

- Prefilled Syringe: 100mg / 5ml
- Ampule: 100mg / 5ml
- Premixed: 1 or 2 grams in 500 of D5W
