Dopamine (Intropin) Actions
1. Increase cardiac contractility
2. cause peripheral vasoconstriction
3. increases chrontropic and inotropic effects
4. dilation of mesenteric and renal artieries promoting urination (reason Dopamine is prefered over norepinephrine)
Low Dose action (less than 10mcg / kg / min
: Beta effects predominate
High Dose action (more than 10mcg / kg / min
: Alpha Effects
Dopamine (Intropin) Indications
1. Cardiogenic Shock
2. Any shock refractory to fluid resusitation
3. Hypotension secondary to bradycardia post resusitation
Dopamine (Intropin) Contraindications
1. Hypovolemic Shock in which fluid resusitation has not been performed
2. Uncorrected tachycardia or VF
Dopamine (Intropin) Precautions
1. ventricular irritability
2. Do not mix with sodium bicarbonate
Dopamine (Intropin) Side Effects
1. VT
2. Hypertension
3. Headache, angina, nausea, vomitting
Dopamine (Intropin) Dosage
2-20mcg / kg / min increased as needed: titrate to level of consciousness, blood pressure, and urine output
Method: 800mg into 500 ml of D5W or NS giving 1600 mcg / ml concentration
