  4. Explain the stepwise management for intermittent symptoms.
    • 1.  Allergen avoidance
    • 2.  Mild: oral antihistamine, Intranasal antihistamine +/- decongestant or LTRA
    • 3.  Moderate/Severe: same as mild or intranasal steroid (or mast cell stabilizer) or allergen-specific immunotherapy
  5. Explain the stepwise management for Persistent symptoms.
    • 1. Allergen avoidance
    • 2. Mild:oral antihistamine, Intranasal antihistamine +/- decongestant or LTRA or intranasal steroid (or mast cell stabilizer) or allergen-specific immunotherapy
    • 3.  Moderate/Severe (in order): intranasal steroid, oral antihistamine, LTRA
    • ADD for Rhinorrhea: ipratropium
    • BLOCKAGE: oral decongestant or oral corticosteroid burst or allergen-specific immunotherapy
  6. MOA: competitive H1 antagonist, or an inverse agonist, of the early response
