Puncture of the outer sac of the heart to get fluid

pericardiocentesis
2 indications:PC
- pericardial effusion
- cardiac tamponade
performed both diagnostically and therapeutically
2 causes of pericardial effusion:PN
- pericarditis
- neoplasia
Heart on x-ray

looks enlarged like glob and the fluid buildup is white
Heart on ultrasound

looks enlarged like a globe and built up fluid is dark
c.s.=LEACSACP
- lethargy
- dyspnea
- anorexia
- collapse
- sudden death
- abdominal distention
- cough
- pleural effusion
Diagnostic imaging

echocardiogram
Materials for pericadiocentesis:BS3CSSCL
- bowl for fluid collection
- syringe
- 3 way stop cock
- central venous catheter (14-16g)
- sterile gloves
- surgical scrub
- collection tubes
- lidocaine

yes
What position are they in?

sternal or R lateral recumbency
What is used to monitor?
- EKG
- During entire procedure!
What is used to dx?

echocardiogram
Patients need to be monitored for:ORD
- ongoing bleeding into the thorax
- recurrent pericardial effusion
- development of cardiac arrythmias
In addition to vitals (HR,RR,BP) what else must be noted?
- pulse stregth
- respiratory effort
