Anti Arrhythmic

  1. PROCAINAMIDE (PRONESTYL) / ANTI-ARRHYTHMIC
    • Procainamide :
    • 1. prolongs ventricular repolarization
    • 2. slows conduction
    • 3. decreases myocardial excitability
  2. PROCAINAMIDE (PRONESTYL) / ANTI-ARRHYTHMIC Indications
    ventricular fibrillation and pulseless ventricular tachycardia refractory to lidocaine
  3. PROCAINAMIDE (PRONESTYL) / ANTI-ARRHYTHMIC Contraindications
    • 1. Hypersensitivity to procainamide or procaine
    • 2. Myasthenia gravis
    • 3. 2nd or 3rd degree heart block
  4. PROCAINAMIDE (PRONESTYL) / ANTI-ARRHYTHMIC Precautions
    • 1. Hypotension
    • 2. Cardiac enlargement
    • 3. CHF
    • 4. AMI
    • 5. Ventricular dysrhythmias from digitalis
    • 6. Hepatic or Renal impairment
    • 7. Bronchial asthma
  5. PROCAINAMIDE (PRONESTYL) / ANTI-ARRHYTHMIC Dosage/Route
    • 20-30 mg/min IV drip
    • Stop points:
    • 1. up to 17mg/kg to effect
    • 2. ectopy resolves
    • 3. QRS complex widens more than 50% from original
    • 4. hypotension ensues
