    Mechanism of Action
    • Slows conduction through myocardium.
    • Elevates ventricular fibrillation threshhold.
    • Suppresses ventricular ectopic activity.

    • V-Tach with a pulse
    • Pre-excited A-Fib
  5. Procainamide

    Contraindications
    • 2nd & 3rd degree heart block
    • Severe conduction system disturbances
    • CHF
    • Should be avoided in Pts with prolonged QT syndrome

    Precautions
    • Monitor CNS toxicity
    • Hypotension (with IV infusion)
    • Monitor BP
    • Hypotensive effects may increase if given with antiypertensive meds
    • Chance of neurotoxicity if given with Lidocaine

  8. Procainamide

    Adult Dose
    20 - 50 mg/min until arrythmia suppressed.

    • * D/C if:
    • Arrythmia suppressed
    • Hypotension
    • QRS complex widened by 50%
    • Total of 17 mg/kg given

    • Slow IV bolus: 20 mg/kg followed by maintenance infusion.
    • Maintenance Infusion: 1 - 4 mg/min
