  1. What is normal Na level?
    • 135-154 mEq/L
    • Intake: ~4-5 g/day
    • Recommended: 2400 mg/day
  2. How can losses of Na occur?
    • GI loss
    • Kidney loss
    • Skin loss
    • Excess ADH
    • Heart failure
    • Diuretics (thiazides)
    • Low Na diet
    • Meds: SSRI, Cytoxan, Lithium, NSAIDs
    • Psychogenic Polydipsia
    • SIADH
  3. Why is D5W questionable for post-op patients?
    Prolonged use for young women can result in hyponatremia
  4. What are S/sx of hyponatremia?
    • Nausea
    • Malaise
    • HA
    • Seizures
    • Coma
  5. What is the Tx for hyponatremia?
    • Fluid restriction
    • Na replacement
    • PO, IV, 3% saline
    • Conivaptan: increases urine output
  6. What is the Tx for SIADH?
    • Correct underlying condition
    • Fluid restriction
    • Demeclocycline: blocks ADH
  7. How much can people lose with diabetes insipidus?
    3-20 L/day
  8. What are S/sx of hypernatremia?
    • Thirst
    • Increase temperature
    • Dry/sticky mucus membranes
    • Lethargy
    • Stupor
    • Cell shrinkage
  9. What are Tx of hypernatremia?
    • Fluids
    • D5W or 1/2 NS gradually
    • Vasopressin for diabetes insipidus - anti-diuretic hormone
