maternal: flushing, drowsiness, headache, lethargy, nausea, vomiting
no effects on neonate
monitor FHR and UC's, serum magnesium levels

prostaglandin synthesis inhibitors
tocolytic drug
depresses synthesis of prostaglandins, effective in delaying delivery 48 hours because of fetal side effects generally used short term and before 32 weeks
maternal: GI upset, serious side effects are uncommon
fetal effects: SERIOUS FETAL SIDE EFFECTS, constriction of the patent ductus arteriosis, neonatal pulmonary hypertension, oligohydramniosis, and intraventicular hemorrhages
monitor FHR and UC's, treat nausea and heartburn

Nifedipine and Nicardipine
tocholytic drug
inhibits smooth muscle contractions of uterus by blocking calcium availability for muscle contraction, delays delivery 48-72 hrs
maternal: flushing, headache, nausea, transient tachycardia and hypotension
fetal: may decrease uteroplacental blood flow
monitor FHR and UC's, maternal BP and HR

ritodrine and terbutaline
tocholytic drug
beta 2 adrengeric effects to suppress uterine activity, can delay delivery for 3 days
maternal: tachycardia, cardiac arrhythmia, myocardial ischemia, and pulmonary edema, elevation in glucose, hypokalemia
fetal: mild tachycardia, IVH
monitor FHR and UC's, I&O for fluid overload, auscultate lungs, monitor HR, glucose

methergine (methylergonovine)
PPH due to uterine atony or subinvolution
Action: stimulates contraction of the uterine smooth muscle
side effects: nausea, vomiting, cramps
check BP, this med can increase the BP

carboprost tromethamine (hemabate)
PPH that has not responded to oxytocin or metergine therapy
action: uterine contractions
side effects: diarrhea, nausea, vomiting, fever
