  1. Appendicitis maneuvers
    • Rovsing: LLQ
    • Psoas: hip extention
    • Obturator: internal rotation
  2. Stess fracture
    • Repetitive insults such as sports leading to complete fracture
    • Most common is of metatarsals
    • Nothing on xray, so CT or MRI
    • Tx is rehab, dec activity, casting, surgery
  3. Compression fracture site
    • 1/3 thoracic
    • 1/3 thoracolumbar
    • 1/3 lumbar
  4. Shoulder dislocation presentation, tests
    • Anterior: side a externally rotated forearm
    • Posterior: medially rotated and held to the side
    • initial- xray, most accurate - MRI
  5. Scaphoid fracture test and tx
    • Xray wont change for 3 weeks
    • Thumb spica cast
  6. Trigger finger tx
    • Steroid injections
    • surgery
  7. Dupuytren contracture
    • Men over 40
    • constricted palmar fascia
    • Surgery
  8. Fat embolism presentation, tests, tx
    • Confusion, petechial rash, SOB
    • PO2 <60, infiltrates on CXR, fat droplets in urine
    • O2 and intubate if neccessary
  9. Signs of knee injuries
    • Ant drawer sign for ACL
    • Posterior drawer from PCL
    • Pop with flex and extend for meniscal
  10. Unhappy triad
    • ACL
    • medial collateral ligament
    • meniscus
  11. Postop fevers
    • POD 1-2: Atelectasis or PNA. Tx with vanc and tazobactam-piperacillin
    • POD 3-5: UTI
    • POD 5-7: DVT, thrombophlebitis
    • POD 7: Wound infection, cellulitis
    • POD 8-15: Drug fever or deep abscess
