Define Category 1 Investigation?
- Conducted by FAO at all accidental fires where MFS/SAPOL Fire Investigation Team is not attending.
- Consists of a Primary and Secondary Investigation.

Define Category 2 Investigation?
- Conducted by FAO at suspicious building fires where Fire Investigation Team is not attending.
- Consists of a Primary Investigation and after consultation with SAPOL, a Secondary Investigation.

Define Category 3 Investigation?
Joint investigation by MFS and SAPOL at all building fires where there is extensive damage or serious injury/fatality.

Define serious injury relating to FCI?
Fire related injury that requires civilian or firefighter to be admitted to hospital.

Who is the lead agency for investigating a deliberate/serious injury fire?
SAPOL.

Who is the lead agency for accidental/undetermined fires?
MFS.

What are the criteria for Fire Investigation Officer attendance/notification? 7 points.
- 2nd alarm A or B risk.
- Request of SAPOL.
- Request of CFS.
- Request of MFS IC.
- Where damage to building and contents exceeds $30,000.
- All fires where Fatality/Serious Injury occurs.
- At request of Chief Officer.

What is the FAO FCI procedure at all fires? 7 points.
- Retain any info taken at scene for legal proceedings.
- Conduct Primary investigation.
- If not suspicious conduct Secondary investigation.
- If cause appears suspicious have SAPOL request CSI.
- If fire caused by electrical appliance/equipment notify FCI section with SP134.
- If fire caused by faulty installation, bad work practice or other preventable action, notify FCI Section during normal business hours.
- Record all details in AIRS.

- Conduct Primary investigation.
- Secure premises.
- Obtain info from owner/occupier, bystanders, FF and anyone else.
- Establish whether premises were secure and account for all MFS forced entry damage.
- Remain in attendance.
- Complete FAO Info form.

- Preserve Scene.
- Interview Witnesses.
- Exterior Examination.
- Interior Examination.

What should be looked for during the Exterior examination of a primary investigation? 4 points.
- Fuel loads.
- Security Breach.
- Anything unusual or suspicious.
- Area of most and least damage.

What should be looked for during the Interior examination of a primary investigation? 7 points.
- Enter at area of least damage.
- Work towards area of most damage.
- Identify fuel loads and account for damage.
- Locate lowest burns.
- Observe V patterns on vertical surfaces.
- Observe overhead damage.
- In low fire load area, lowest point of burning below most overhead damage may be precise point of origin.

Define Secondary Investigation?
Confirming possible/probable cause of non-suspicious fires by conducting any necessary overhaul in the area of origin in an attempt to identify the source of ignition, fuel ignited and circumstances.
