Causes of Infectious Endocarditis
- Causes:
- Staph, Strep (bacteria Tx w/antibiotics 4-6wks)
- Viruses
- Fungi
- Rheumatoci disease.
- Procedure:
- Dental: dislodges bacteria in mouth--> heart.
- OB: childbirth
- GI
- Valve replacement (if valve is contaminated during sx.)
- Marfan's Syndrome
All nfxn-->platelet/fibrin and vegetation aggregation in heart.
-
Assessment of infectious endocarditis
- murmer (apart from sound, SnSs resemble HF with valvular disorder).
- ^BR
- ^Temp
- ^WBC
- anorexia
- malaise
- Splinter hemorrhage (looks like splinters in nails, can also be striations)
- Osler’s nodes (spots noted on hands and feet)
- Janeway’s lesions (spots noted on hands and feet)
- Clubbing
-
Treatment
- antibiotics (4-6wks)
- diet
- pain Rx
- Sx
- antipyretic
w/treatment ~70% of patients recover.
-
Pericarditis
- Inflammation of the pericardial sac
- Causes:
- Idiopathic (unknown cause)
- Viruses
- Bacteria
- Fungi
- AMI
- Cancer
- Trauma (including Sx)
-
Assessment of pericarditis
- Pleuritic chest pain
- Dyspnea
- Pericardial friction rub*
- anxiety
-
Intervensions for pt w/pericarditis
- Tx underlying cause
- Antibx?
- Corticosteroids?
- ASA/NSAIDS?
- Bedrest
- Pericardiocentesis?
-
Complications associated with pericarditis
- *Pericardial effusion
- --Accumulation of fluid in pericardium
- --Distant/muffled heart tones
- Cardiac tamponade
- --Increased intrapericardial pressure 2o to large pericardial effusion
- --Confused, agitated & restless
- --Tachycardia & tachypnea
- --JVD
- --Pulsus paradoxus
