birth; 2, 4, & 6 mos

after 1st b-day & 6 mos later

3 doses total @ 2, 4, & 6 mos (must be given prior to 32 weeks of age) 

2,4,6,12mos; age 4 or before school

2,4,6,12 mos

2 mos, 4mos, 6 or 12 mos, & 4th dose at age of 4

after 6 mos and yearly thereafter

after age 1; age 4

12 mos; age 4

24 mos through age 18 (not given routinely but remember this is for college students living in a dorm so they get it before that)

9 to 26 yrs of age; 3 doses total; give first dose; 2 mos later give 2nd dose; 6 mos later give 3rd dose
