To Speech- eyes open with verbal stimulation but not necessarily to command

To Pain - eyes open with nouxious stimulation

None- no eye opening regardless of stimulation

Oriented - accurate information about person, place, time, reason for hospitalization, and personal date

Confused - answeres not appropriate to queston, but use of language is correct

Inappropriate words - disorganized, random speech, nossustained conversation

Incomprehensible sounds- moans, groans, and incomprehensible mumbles

None - no verbalization despite stimulation

Obeys commands - perfoms simple tasks on command; able to repeat performs

Localizes to pain - organized attempt to localez and remove painful stimuli

Withdraws from pain - withdraws extremity from source of painful stimuli

• Decorticate posturing
•                     Spontaineously or to noxious stimuli

• Decerebrate posturing
•                     spontaneously or to noxious stimuli

None - no response to noxious stimuli
