characteristics of conductive hearing losses 7
no tolerace problems
no word recognition difficulties
paracusis willsii
low frequency tinnitus
generally all frequencies affected
autophony
muffles sounds
outer ear conditions 4
blockage
congenital atresia
otitis externa
exostosis
paracusis willsii
hear better in noise because everyone around them is yelling over the noise
low frequency tinnitus
hearing very low sounds such as heart beat, fluttering, etc
autophony
hearing yourself too loud
blockage
no big deal
caused by wax
congenital atresia
birth defect
no ear canal
otitis externa
external inflamation
fungus infection
exactly like athletes foot
NOT swimmers ear
exostosis
swimmer/surfer's ear
caused by swimming in cold salt water
no real problem
no real treatment
microtia
deformed pinna / no pinna
macrotia
pinnas are too large
treatments for outer ear disorders 4
remove blockage (cerumin) - look at state laws
surgery for atresia - AuD do not perform
medicines for infections
exotosis are generally left alone
common middle ear disorders
cancer of the middle ear
glomus tumor
myringitis bullosa
pagets disease
Van der Hoeve's Syndrome
Trauma
cancer of the middle ear 4
happens a lot in diabetics
looks like otitis media but does not go away
is treatable
runs the risk of becoming cancer in the bone then the brain
glomus tumor 4
first sign is hearing the heart beat constantly
hits the ossicular chain or tympanic membrane
can be medically treated
sunrise tympanogram
As
Ad
Au
left ear
right ear
both ears
myringitis bullosa 3
viral infection of the Tympanic Membrane
inflammation of ear drum
herpies infection
paget's disease
disease of the elderly
genetic, congenital
major bones of body swell and twist
Beethoven
Van der Hoeve's syndrome 3
osteogensis imperfecta
brittle bone disease
genetic
trauma 4
most common problem especially for our age group
common until 24
any blow to te head can create a hearing loss
if they dont lose consciousness then they probabky did not get a hearing loss from trauma
define tympanoplasty
reconstructive surgery of the middle ear
4 types of tympanoplasty
1 - patch a hole in tympanic membrane
2 - connect TM to Incus (because malleus is missing)
3 - connect TM to Head of stapes (malleus and incus missing)
4 - connect TM directly to oval window (fenestration)
otosclerosis 4
genetic disease
2:1 female
: male
typically white women from 15-45 (child bearing years)
pregnancy does not cause but can lead to expression of the gene
otosclerosis problem?
bone grows around the oval window or round window creating a hearing loss
otosclerosis looks
rose colored = beautiful
treeatment of otosclerosis
medical - sodium flouride (not proven to work)
surgical -
fenestration - no
stapes mobilization - temporary
stapedectomy / stapedotomy - yes
steps in stapedotomy
measure stapes height
drill hole in bottom
cut crewa
cut stapes tendent
break stapes out
put in fake stapes (made of teflon cause bone cannot stick to it)
stapedotomy chance of cure
99%
if it stops working, it can be done again or you can have hearing aids
stapes mobilization
removes bone around stapes which fixes it for about a year until the bone grows back
fenestration 4
completely reconstructing the ear
only 50% chance of improvement
makes patient dizzy for awhile
problem when doing middle ear molding
