Analgesic Meds

  1. Morphine
    • Uses: Severe pain, sedation, pain & anxiety of AMI, acute pulmonary edema
    • -analgesic, narcosis

    • ADE: Peripheral vasodilation, nausea, constipation, CNS depression, Miosis, blur (due to parasympathetic)
    • -tolerance and addiction can occur
    • -CYP 450 inducer
  2. Hydromorphone (Dilaudid)
    Uses: analgesic, narcosis

    • ADE: CNS depression, sedation, cardiac changes, dizzy, itchy, constipation, nausea, urinary retention, blur, diplopia, miosis, nystagmus.
    • -tolerance and addiction can occur
  3. Oxycodone (OxyCotin)
    Uses: moderate to severe pain

    • ADE: CNS depression, sedation, cardiac changes, dizzy, itchy, constipation, nausea, urinary retention, blur, diplopia, miosis, nystagmus.
    • -Dependence can occur
  4. Hydrocodone (Vicodin, Norco)
    Uses: Moderate pain - alternative to Codeine

    • ADE: CNS depression, sedation, cardiac changes, dizzy, itchy, constipation, nausea, urinary retention, blur, diplopia, miosis, nystagmus.
    • -Dependence can occur
    • -used under co-mgmt of surgery & trauma to the eye.
  5. Codeine
    • Pro-drug, 10% metabolized to morphine.
    • Uses: moderate pain, anti-tussive agents.
    • -combined with APAP or ASA for analgesic synergy.

    • ADE: CNS depression, rash, dizzy, itchy, constipation, nausea, blur, diplopia.
    • -Dependence can occur
  6. Fentanyl
    (Fentanyl, remifentanil, sufentanil, alfentanil)
    • More potent than marphine
    • Uses: preoperative/anesthetic agents, severe pain

    • ADE: CNS depression, sedation, cardiac changes, dizzy, itchy, constipation, nausea, urinary retention, blur, diplopia, miosis, nystagmus.
    • -Dependence can occur
  7. Tramadol
    MOA: Binds to Mu-opiate receptors in the CNS & cause inhibition of ascending pain pathways.

    Uses: Moderate to severe chronic pain. (PO only)

    • ADE: HA, sedation, dizzy, itchy, constipation.
    • -doses adjusted for renal/hepatic dysfunction
  8. ASA (Asprin)
    Uses: Mild/moderate pain, fever reducer, reduces inflammation, reduces blood clotting, uricosuric agent.
  9. APAP, Acetaminophen (Tylenol)
    MOA: Inhibits synthesis of prostoglandins in the CNS and Peripherally blockspain impulse generation, inhibits hypothalamic heat-regulating center.

    Uses: mild-moderate pain, reduces fever. (no anti-inflammatory effects)

    • ADE: Metabolized hepatically (overdose can cause hepatotoxicity-most common for accidental toxicity)
    • -Don't exceed 3gm/day - FDA
  10. (Migrane Med)
    Sumatriptan
    Rizatriptan
    Almotriptan
    5-HT1 (Seratonin) receptor agnoists
    "Triptans"
    Uses: Migrane Tx (vasoconstiction to reduce inflammation)

    ADE: Inc BP, chest pain, Palpitations, vasospasm, dizzy, drowsy, abdominal pain.

    -cautions in persons with coronary artery disease.
