- attached to the beginning of a word. It usually, but not always,
- indicates location, time, number, or status. A common prefix is dys-, meaning painful or difficult.

- It usually, but not always, indicates the procedure, condition,

The word root usually, but not always, indicates the part of the body involved.

The combining form is created by adding the letter "o" to the end of a word root. As its name suggests, the combining form allows a root word to be combined with other word roots, combining forms, or suffixes to make the medical term. A common word root is splen, which means spleen. Adding the "o" at the end makes this a combining form, splen/o.
