  1. Dilated CMP:
    LV enlarges; Contractility stays the same

    • caused by:
    • Ischemia
    • inflammation
    • toxins

    HTN, valve disorders contribute
  2. Dilated CMP:
    Systolic or Diastolic?
    Affects systolic function

    • results from damaged heart muscle fibers
    • LV contractility decreases
    • Cardiac output decreases

    Sympathetic NS stimualated to increase HR and contractility
  3. Kidneys Kick in:
    • Retain:
    • sodium
    • water

    • Maintains cardiac output
    • vasoconstriction
    • renin-angiotensin system

    heart fails when these mechanisms no longer compensate
  4. Detrimental Dilation
    • LV dilation occurs
    • Atrial also dilates
    • More work to pump blood==

    • results in CARDIOMEGALY
    • BLOOD POOLS==EMBOLI
  5. Signs/Symptoms
    • S3-S4 gallop
    • SOB
    • Fatigue
    • Dry cough (Left sided failure)
    • Peripheral edema
    • JVD
    • weight gain.
  6. Treatment:
    Treat underlying cause

    • ACE Inhibitors First Line
    • Diuretics
    • Digoxin
    • Anticoaguants
    • AICD
    • Amiodarone
