IE

  1. streptococcus MIC < 0.12 (pen susceptible) w/o valve
    • penicillin 12-18 units/day x 4 weeks
    • OR
    • ceftriaxone 2g/day x 4 weeks

    (use vanco if pen allergic)
  2. streptococcus MIC < 0.12 (pen susceptible) with valve
    • penicillin 24 units/day x 6 weeks
    • OR
    • ceftrixone 2g/day x 6 weeks

    (use vanco if pen allergic)
  3. streptococcus MIC >0.12 < 0.5 w/o valve
    • penicillin 24 units/day x 4 weeks + gentamicin 1mg/kg q8h x 2 weeks
    • OR
    • ceftrixone 2g/day x 4 weeks + gentamicin 1mg/kg q8h x 2 weeks

    (use vanco if pen allergic)
  4. streptococcus with valve OR MIC >0.5 (pen resistant) with or w/o valve
    • penicillin 24 units/day x 6 weeks + gentamicin 1mg/kg q8h x 6 weeks
    • OR
    • ceftrixone 2g/day x 6 weeks + gentamicin 1mg/kg q8h x 6 weeks

    (use vanco if pen allergic)
  5. enterococci
    • penicillin 18-30 units/day + gentimicin 1mg/kg x 4-6 weeks
    • OR
    • ampicillin 2g + gentimicin 1mg/kg x 4-6 weeks

    • treat 4 weeks with native valve and sxs <3 months
    • treat 6 weeks sxs last > 3 months

    (in pen allergic use vanco and treat for 6 weeks)
  6. staphylocci w/o valve
    • nafcillin 2 g q 4h x 6 weeks
    • OR
    • oxacillin 2 g q 4h x 6 weeks

    • (if pen allergic use cefazolin or vanco for nafcillin)
    • (MRSA give vanco)
  7. staphylococci w valve
    • nafcillin 2 g q 4h x 6 weeks + rifampin x 6 weeks + gentamicin x 2 weeks
    • OR
    • oxacillin 2 g q 4h x 6 weeks + rifampin x 6 weeks + gentamicin x 2 weeks

    • (if pen allergic use cefazolin or vanco for nafcillin)
    • (MRSA give vanco)
  8. HACEK organisms
    ceftriazone x 4-6 weeks

    • 4 weeks w/o valve
    • 6 weeks w/ valve
  9. Endocarditis Prophylaxis
    oral
    amoxicillin 2g or 50mg/kg
  10. Endocarditis Prophylaxis
    NPO
    • ampicillin 2gIM/IV or 50mg/kg IM/IV
    • OR
    • Cefazolin/ceftriaxone 1gIM/IV or 50mg/kg
  11. Endocarditis Prophylaxis
    Allergic to Penicillins/ampicillins
    • Cephalexin 2g or 50mg/kg
    • OR
    • Clindamycin 600mg or 20mg/kg
    • OR
    • Azithromycin/clarithromycin 500mg or 12mg/ks
  12. Endocarditis Prophylaxis
    allergic to penicillins/ampicillins and NPO
    • cefazolin/seftrixone 1g IM/IV or 50mgkg
    • OR
    • Clindamycin 600mg IM/IV or 20mg/kg
