- Salmeterol
- Formoterol
- Arformoterol
MoA
- Beta 2 receptor stimulation
- Attaches to a G protein-linked receptor
- Stimulates adenylyl cyclase increasing cAMP
- Also stimulates Na+-K+ ATPase pump and insulin release
Use
- Exercise-induced bronchospasm
- Asthma control
- Do not use alone
- No tachyphylaxis
Effects
- Similar to SABA’s but minimal anti-inflammatory effects
- or mucociliary clearance.
- Less useful for K+ movement due to slower onset of action.
Major SEs
- Headaches
- Musculoskeletal pain
- Carries a “black box” warning for increased asthma deaths, particularly among
- African-American population
- Should not be used for acute symptoms
- Should not be instituted in patients with significantly worsening asthma or acute exacerbations, due to reports of severe respiratory events – deaths
Contraindication/Drug interactions
- Contraindications – as above.
- Drug interactions – beta blockers may reduce effect.
