-
Organism
- Similar to campylobacter
- Urease allows to exist in low pH
- Vacuolating cytotoxin causes apoptosis -> large cytoplasmic vacules
- Pathogenicity islands
- Cag: injected protein associated with virulence
-
Epidemiology
- Very prevalent
- Fecal-oral
- Strain persists for decades
- Class one carcinogen
-
Pathogenesis
- Adheres to gastric mucosa
- Motility provided by flagella -> swim to less swim to less acidic pH beneath mucosa
- Urease
- Adhesion mediated by surface proteins
- Cellular infiltrate -> microabcess formation
- Vacuolating cytotoxin -> epithelial cell erosion
- Cag -> reorganization of cytoskeleton
-
Manifestations
- Silent or nausea, upper abd pain
- Gastritis, peptic ulcers -> nausea, vomiting, epigastric pain
-
Diagnosis
- Endoscopic examination
- Culture
- Potent urease activity -> one hour to detect
- IgG, IgA for screening/evaluation
-
Treatment
Bismuth-salts with two antibiotics
